Corewell Health offers a variety of resources for patients and community looking to manage and prevent diabetes. Corewell Health has five outpatient diabetes self-management education and support centers that are certified by the Michigan Department of Health and Human Services and recognized by the American Diabetes Association for quality self-management education.

In addition to the myriad of services that the outpatient diabetes centers provide, Corewell Health offers community based programs to help support diabetes management and prevention. Below is a breakdown of various types of diabetes prevention and management services offered, as well as, resources and tools to help you manage your health and future well-being.

Diabetes Prevention Program

Prediabetes is a health condition where blood sugar levels are higher than normal. These levels are not yet high enough to be diagnosed as type 2 diabetes. Approximately 84 million American adults currently have prediabetes. Of those with prediabetes, 9 out of 10 people don’t know they have it. Prediabetes puts you at increased risk of developing type 2 diabetes, as well as, heart disease and stroke. It’s important to talk to your provider about getting your blood sugar tested if you have any of the risk factors for prediabetes, which include:

being overweight, BMI of 25 or higher
being 45 years or older
having a parent, brother, or sister with type 2 diabetes
being physically active less than 3 times a week
having hypertension or high blood pressure
having gestational diabetes (diabetes during pregnancy) or a 9 pound (or more) baby

A year long lifestyle change program nationally recognized by the Centers for Disease Control and Prevention, called the Diabetes Prevention Program, can help you prevent diabetes by making changes over time. Type 1 Diabetes

Type 1 diabetes is an autoimmune disease which means the beta cells in the pancreas are mistakenly destroyed by the body’s immune system. The beta cells produce insulin, so without these cells a person’s blood glucose (sugar) is too high.

Symptoms of Type 1 diabetes can include: increased thirst, frequent urination, feeling hungry even though you are eating, extreme fatigue, unexplained weight loss, dry mouth, itchy or dry skin.

All people with Type 1 diabetes require insulin, but a healthy lifestyle is also important and that is where Corewell Health can help. Whether you are an adult or child with Type 1 diabetes, Corewell Health has resources for you.

Type 2

If you have Type 2 diabetes your body does not use insulin properly. The pancreas initially makes extra insulin to make up for it. However, over time the pancreas is unable to keep your blood sugar in a normal range. In many cases, you can control diabetes through nutrition, a healthy weight, physical activity and medication. Education is an important tool in the day to day management of Type 2 diabetes.

DPATH (Diabetes Personal Action Toward Health) is a 6-week workshop for adults with Type 2 diabetes, their families, friends and caregivers. The workshop is led by two trained leaders and is focused on techniques to deal with symptoms of diabetes, appropriate exercise for maintaining and improving strength and endurance, healthy eating, working with health care providers, problem solving and building an action plan. Gestational

Gestational diabetes (GDM) is a type of diabetes that occurs during pregnancy where a woman experiences an intolerance to carbohydrates. All pregnant women should be screened for GDM with a laboratory-based test. Education for GDM consists of either a single group class or individual education, depending on the needs of the patient. In the program, you’ll learn about managing gestational diabetes including nutrition, physical activity, glucose monitoring, and medications. The program is taught by certified diabetes care and education specialists and is certified by the Michigan Department of Health and Human Services and recognized by the American Diabetes Association.

Medical Nutrition Therapy

Medical nutrition therapy (MNT) is a visit with a registered dietitian who will work with you to treat and prevent disease through an individualized nutrition plan. Our dietitians work with people who have diabetes and pre-diabetes as well as other health conditions.
