Whether you need primary care, a specialist, a specific service or support, getting you connected with the care and resources that are right for you is our goal. That’s why we have a patient navigator dedicated to the LGBTQIA+ community who can match you with a safe and affirming provider that fits your needs.

Our patient navigator is also embedded in the community, developing relationships with support, social and advocacy groups to ensure you can get connected with the right resources when you need them. Contact the patient navigator at 833-968-0263 (Monday to Friday, 8 a.m. to 5 p.m.) or complete the form to have the navigator contact you.
