At Corewell Health Sleep Clinic - 5452 Fort St, we offer services for Sleep Medicine Services associated with Sleep Disorders. Patients can expect compassionate, extraordinary care provided by a professional medical staff member. Our practice has partnered with other specialists that operate within the same location. The specialists include an orthopedic surgeon, Bariatric surgeon as well as a Gastroenterologist.  

Before you arrive

• New patients should complete paperwork mailed from our clinic prior to their appointment. We ask patients to arrive 15 minutes prior to their appointment to be pre-registered at check in. 
• Arrive with valid ID and Insurance card(s).
• Patients who require an authorization/referral for insurance purposes will need to do the following:
  • Request an authorization/referral from their primary care physician or referring physician.
  • Call our office one week prior to their scheduled appointment to insure we have received the required referral

• park in Lot 1
• Handicap parking available close to the main hospital entrance.
• Once you are parked, enter through the main hospital entrance and take the first elevator to your right (across from the service desk) up to level 2 and suite 200 is immediately to your left.
